Notes on Health and the Things We Measure

Health is rarely maintained alone, and it is frequently maintained on behalf of someone else. Parents, partners, adult children, and friends carry a substantial part of the burden of another person's wellbeing, usually without recognition and often at cost to their own.

Where habit meets circumstance, what emerges is a description of one's own operating conditions, which is worth more than any general recommendation because it is actually about the person following it.

Considered plainly, there is a further point, less regularly made. The relationship between health and care runs in both directions. Being needed sustains people; purpose is protective. Isolation, not obligation, is the greater danger. The goal is not to be free of others but to be attached to them in a way that does not require self-erasure.

In practice prevention has several layers. There are behaviours that shift risk across an entire population over decades: not smoking, moving regularly, sleeping adequately, drinking moderately or not at all, eating in a way that includes plants and does not consist mainly of ultra-processed food. There is early detection, which changes the nature of a disease rather than its existence — screenings, dental examinations, eye tests, blood pressure taken occasionally rather than never. There is vaccination, which prevents the disease outright. And there is the maintenance of the conditions that make all of this possible: sufficient money, sufficient sleep, and enough mental stability to attend an appointment.

Whatever else wellness consists of, it is not a solitary achievement. It is produced between people, and its costs and benefits are shared whether or not anybody has agreed to it.

Self-observation, conducted with a minimum of rigour, is therefore valuable. Not the continuous surveillance of a device, but the periodic noticing of pattern. Which days end with energy remaining, and what did they contain? Which meals precede an afternoon of clarity, and which precede a slump? How many hours of sleep are required before irritability disappears — an amount most people can identify but few have ever established. What happens to mood after two weeks without exercise? After a weekend alone? After alcohol?

These questions have answers, and the answers are personal. Some people function on six hours; most who believe they do are wrong. Some tolerate caffeine in the afternoon; many do not and have never tested it. Some are lifted by solitude and drained by company; for others the reverse.
