Notes on Health, Work and the Modern Schedule

There is a distinction between exercise and physical activity that has turn into important as work has become sedentary. Exercise is a bounded event: forty minutes, a defined place, a change of clothes. Physical activity is everything else the body does. For most of human history the second was substantial and the first did not exist.

Looking at the evidence over decades, air quality, damp, mould, and noise have measurable effects on respiratory health and sleep and are frequently tolerated far longer than they should be.

Looking at what shapes daily health, recognising the power of environment does two things. It reduces the moralising: people living in circumstances hostile to health are not failing at self-control. Space for movement need not be a gym. A clear patch of floor, a chin-up bar in a doorway, or a bag of something heavy is enough to make a five-minute intervention possible on a day when leaving is not.

When we examine daily patterns, the two together describe a reasonable picture: a day with motion distributed through it, and a small number of sessions in which the whole self is asked to do something demanding.

Considered plainly, individual choices receive most of the focus in discussions of health, but choices are made inside environments, and environments do a great deal of the deciding. The air a person breathes, the distance to green space, the presence of pavements, the price of vegetables, the noise at night, the security of employment — all of these shape health outcomes without passing through anybody's intentions.

This is encouraging, because interrupting sitting is available to almost everyone. Standing during phone calls. A short walk after each meal, which blunts the post-meal glucose rise. Stairs. Work environments exert enormous influence. Shift work disrupts circadian rhythm in ways that no personal habit fully offsets. Sedentary jobs demand deliberate compensation.
