Understanding Time, Attention and Health

There is a distinction between workout and physical movement that has become central as work has become sedentary. Exercise is a bounded event: forty minutes, a defined place, a change of clothes. Physical activity is everything else the organism does. For most of human history the second was substantial and the first did not exist.

Prevention suffers from an awkward feature: when it works, nothing happens. There is no gratitude for the heart attack that did not occur, no relief at the cancer detected early enough to be dull. The reward for prevention is an absence, and absences are difficult to feel.

The two together describe a reasonable picture: a day with motion distributed through it, and a minor number of sessions in which the body is asked to do something demanding.

In practice prevention has several layers. There are behaviours that shift risk across an entire population over decades: not smoking, moving regularly, sleeping adequately, drinking moderately or not at all, eating in a manner that includes plants and does not consist mainly of ultra-processed food. There is early detection, which changes the nature of a disease rather than its existence — screenings, dental examinations, eye tests, blood pressure taken occasionally rather than never.
